The aim of the workshop is to present a method for analysing individual railway hazard log entries to assign and map appropriate human components and to relate them to Human Factors project activities.
The method is being used in practice on a re-signalling programme and has been embraced by the safety management team and included in the hazard management plan. The method uses human components to map many aspects of the programme, including:
• HF record of activities and reporting to the ISA;
• HF issues;
• Hazard repository;
• The communications connections in the command and control organisation;
• Operational testing;
• Training modules;
• Operational rule changes.
The method provides opportunities for the use of a structured approach, which has a key advantage of providing a mechanism to present a “completeness” argument. The method provides the opportunity for grouping, sorting and filtering of items such that inaccuracies and errors in recording can be more readily identified.
If recorded within a relational database, such as DOORS, the method allows for integration of content and the production of reports that can satisfy answers to questions such as:
• If this test fails because the User cannot implement an operational rule in the changed system, which hazards cannot be closed?
• What HF evidence do we have that satisfies all hazards associated with a particular piece of equipment?
• Why is the usability of a specific user interface important (in terms of specific which rules, hazards, training are affected)?
• If the project wants to import a hazard to the programme, what does this mean in terms of the human components and the related areas of training, rules, operations, etc.?
This workshop will include a number of practical worked examples, including the opportunity for participants to complete mapping activities themselves. There will be an opportunity to ask questions about the practical application and lessons learnt. The workshop will cover the use of the method in application at both system-level and product-level.
This workshop is estimated can fill 1.5 to 2.5 hours, and will include:
• Interactive presentation with the authors;
• Working through worked examples using worksheets and presentation slides;
• The opportunity for participants to leave with their own map to start using in the context of their HF and hazard logs;
• Time for questions and answers.
