The effectiveness of Pre operative full liquid diet course in obesity surgery

Objective: In order to reduce the amount of visceral fat prior to bariatric surgery a preoperative very low calorie diet (VLCD) is commonly employed. Duration often depends on degree of obesity and desired amount of weight loss. Patient compliance, tolerability and success are not always known. Aim of this study is to evaluate compliance and efficacy of this regimen.
Method: One hundred and one patients undergoing obesity surgery were asked to follow a 800 kcal full liquid diet. The VLCD was in the form of a commercially available product. Patients with a BMI <50 (group A) were instructed to be on the diet for one week and those with a BMI >50 (group B) two weeks. Weight loss was calculated and 4 domains assessed using a 4-point scale for hunger, satisfaction, deviation and desire for solids. Primary outcome was amount of weight loss. Secondary outcome was compliance and satisfaction.
Result: 70 patients in Group A and 31 patients in Group B had a mean weight loss was of 9.57+ 4.06 and 14.74+ 5.04 lbs. respectively. Hunger was 2.01 and 2.42 for groups A and B respectively. Satisfaction was 3.36 and 3.29 for groups A and B respectively. 20 % in group A and 19 % in group B consumed some form of solid food.
Conclusion: Pre operative full liquid diet provided moderate amount of weight loss in both groups with similar hunger and satisfaction scores. Correlation with intraoperative degree of visceral fat loss and technical ease should be studied.
